Friends and Family Access

Information for Designating Friends and Family Access for BCH Services

Friends and Family access lets an individual view the medical records of another person and make medical decisions for that person. Friends and Family access is relevant for a parent or legal guardian of a child; the legal guardian or holder of a health care power of attorney for individuals age 18 or older; or another legally recognized Personal Representative. A Personal Representative is a person who has the authority under state law to consent to treatment on behalf of the patient; including court-appointed guardians and parents on behalf of unemancipated minors.

There are three ways to establish a Friends and Family Access for MyBCH:

1. A patient can use MyBCH to invite someone else (family member or friend) to have Friends and Family access to their record. The other person must also have a MyBCH or MyChart account with Epic. In the online portal or app, this can be found under Menu > Sharing > Sharing Hub > Family member, close friend, or caretaker > Manage ongoing access to your MyBCH account.

2. Our Friends and Family Access forms are located above by selecting the links or can be found by going to the my.bch.org login page. You will need to click on the Request Proxy Access button to access the forms. If a person wants to set up proxy access for a legal relationship (Power of Attorney, Non-Parent Guardian, etc.), you must submit legal documentation to the BCH Health Information Management (HIM) department. Documents and completed forms can be emailed to MyBCH@bch.org or sent via the MyBCH portal by sending a customer service or proxy access requests message with the uploaded form.

3. Friends and Family Access forms are also available at each BCH clinic. Please request a Friends and Family access form from the clinic receptionist. They will be reviewed and access will be granted by the HIM department. For information regarding a Friends and Family access request, a patient can call HIM at 303-415-7760 (option 3), Monday-Friday, 8 a.m. - 4:30 p.m.
