Compassionate, specially trained health care team
Our geriatric team has advanced training in caring for aging adults, as well as those living with the challenges of multiple medical conditions. Our team can be helpful to you and your family if you:
- Have multiple chronic conditions such as memory loss and dementia, osteoporosis, incontinence and heart disease
- Experience functional decline or physical frailty
- Take multiple medications for chronic conditions
- Require appointments with and treatments from different providers
- Are recovering from a hospital stay
Support for family members participating in your care
We also make it a priority to support family members who help care for you by providing easy access to your health information. For instance, through our MyBCH patient portal, family members can message your providers, check test results and participate in your care.
Referral process
Geriatric Medicine of BCH requires referrals from a primary care provider. Referrals are reviewed for appropriateness prior to scheduling.
