BCH Foundation's Heart to Heart fund provides financial support for cardiology patients

The Boulder Community Health Foundation's Heart to Heart fund has been helping patients afford cardiology care since February of 2023. This fund provides financial support for Boulder Heart cardiology patients in the prevention, treatment, and management of their cardiac health. When nominated by a care provider, patients become eligible to receive a grant to help support supplemental health needs.
Possible funding opportunities, determined by the care team at Boulder Heart, may include but are not limited to:
- Support for cost of transportation to and from appointments
- Support for co-pays, clinical, and medication costs
- Support for the cost of medical equipment like blood pressure cuffs and cardiac monitors
- Access to integrative care and wellness services like nutrition, fitness, massage, acupuncture, and reiki
- Access to behavioral health care
The donors who established the program hope that Heart to Heart will continue to provide financial assistance to cardiac patients so they have one less worry and can focus on their health, wellness, and recovery. In 2024, 100 Heart to Heart grants were given to Boulder Heart patients totaling $60,731 in support. Most common diagnoses for receiving Heart to Heart funding is Coronary Artery Disease, Heart Failure and Atrial Fibrillation.
