BCH Foundation State of the Union 2024

On February 29, 2024, community members and donors gathered in the Leo Hill Conference Room at Boulder Community Health to hear from BCH President and CEO, Dr. Robert Vissers and Vice President of Community Health & Engagement and BCH Foundation President Grant Besser during our State of Union event. In his address, Dr. Vissers shared success stories from BCH in 2023 and shared plans of growth for 2024 and beyond. Watch the full conversation below:

Following the address from Dr. Vissers, Madelyn Hunt, Manager of Case Management Services at Optum serving BCH presented a case study on complex patients through our Project HEALS program. Complex Case Management involves assessing and coordinating care for patients often with diverse health needs, encompassing physical, mental, and psycho-social aspects, which may result in barriers to discharge from the hospital setting. Project HEALS helps BCH address barriers to discharge, enhancing patient health & well-being through comprehensive partnerships & programs. Watch Madelyn’s full presentation below:
