Provides financial assistance to cardiac patients so they have one less
worry and can focus on their health, wellness, and recovery. The grants
made from this fund support patients in the prevention, treatment, and
management of their cardiac health.
Cardiology Nurse Navigator
Nurse navigator role within the Boulder Heart team, to provide navigation
of services and additional support for patients identified within a specific
set of parameters. This role increases preventative support services,
reduces patient barriers, and ensures BCH remains a health system of choice
for cardiac related illnesses in Boulder County.
Cardiology Support Classes
Patients receiving care at BCH’s Boulder Heart are supported through
a variety of classes, including nutrition, meditation, cooking, lifestyle
management, and caregiver support. These programs are designed to help
patients and their families stay committed to their treatment plans, enhance
their quality of life, foster a sense of community, and ultimately save lives.
Boulder County Public Health Partnership for Fruit & Veggie Program
Boulder patients receiving care at BCH’s Boulder Heart and enrolled
in Medicaid are eligible to receive up to $80 a month in fruit and vegetable
coupons. These can be used at various grocery stores and farmers' markets
throughout Boulder, making it easier to access fresh, nutritious produce.
