Project Angel Heart

In partnership with Project Angel Heart, BCH arranges the delivery of nutritionally tailored meals to recently discharged patients facing food insecurity. This initiative allows patients to focus on their recovery, alleviates

anxiety for both patients and their families, and ensures that the meals meet dietary needs based on medical recommendations.

Project HEALS

This project allows the Case Management team to help care for patients experiencing barriers to post-acute care because of several factors, including housing, behavioral health, traumatic brain injuries, dementia, and other social and economic barriers. A portion of the grant will fund a bed at a respite care facility run by the Colorado Coalition for the Homeless to assist in providing post-acute care to individuals who are too medically vulnerable to return to the streets or All Roads, but no longer require hospital inpatient care. The second part of the grant will provide patients with in-home care for up to 10-days, four hours per day, to assist in the transition from inpatient to being home.
