The Boulder Valley Care Network (BVCN) is an Accountable Care Organization (ACO) comprised of more than 600 of the area’s finest physicians and advanced practice providers (APPs) from Boulder Community Health and the Boulder Valley Individual Practice Association (BVIPA). BVCN providers work together to coordinate your health care — all with the goal of helping you become and stay healthier.
What are the benefits of an Accountable Care Organization?
An ACO is a clinically integrated network of health care providers that
share goals for quality of care, value and health outcomes. Being part
of the BVCN means that your care team is able to:
1. Enhance Your Care Coordination
The BVCN connects your primary care and specialty care providers within
one network so your doctors, physician assistants, nurse practitioners
and other specialists are able to work together to enhance and better
coordinate your health care. BVCN providers have access to your most up-to-date
health information, clinical guidelines, resources and tools.
2. Identify Those Whose Health Might Be at Risk
Patient data analysis by the BVCN helps identify those patients who might
be at risk for chronic health conditions and could benefit from coordinated
care. Through a cooperative effort, our providers then share their expertise
and identify resources to develop a care plan to help keep these patients
healthy and avoid emergency department visits and hospital admissions.
3. Deliver Better Access to Care
The BVCN helps patients find the medical providers closest to them who
can best meet their medical needs. In turn, our providers can collaborate
with each other when a patient’s care requires a multi-disciplinary
skill set.
4. Reduce Health Care Costs
Our enhanced care coordination helps achieve better health outcomes that
is more cost efficient. A coordinated approach to care helps reduce duplicate
tests, unnecessary interventions, emergency department visits and hospital
readmissions.
5. Improve Health Outcomes
BVCN providers collaborate to meet established care quality measures such
as health outcomes and patient experience.
