Boulder Heart at Community Medical Center (CMC) is a convenient option for our BCH patients living in Boulder, Broomfield, Erie, Lafayette, Louisville and Superior.
At the Boulder Heart office in Lafayette, our highly experienced cardiologists and surgeons build lasting personal relationships with each patient. We listen to you, we know your history, and we partner with your other medical providers to provide the right cardiac care at the right time. We are the Boulder Valley’s only full-service cardiology program, providing everything from diagnostic testing to the latest advanced treatments.
The CMC building is home to Lab, Imaging, Primary Care and an Emergency Department — all of which may be needed for our community members during cardiac treatment.

Hand &Upper Extremity Therapy
Our certified hand therapists (CHT) with Outpatient Rehabilitation can help you heal and regain maximum use of your injured hand or an upper extremity. They have advanced training and experience in the treatment of hand, wrist, forearm and upper arm conditions.

Our board-certified pathologists provide a full range of standard and specialty tests in partnership with Mayo Clinic Reference Lab. Self-pay pricing is available.

Occupational Health Services
As a board-certified specialist in physiatry (physical medicine), Lynne Fernandez, MD has extensive training and expertise in treating musculoskeletal problems, helping patients control pain, and designing rehabilitation regimens. She directly treats patients with complicated medical issues, and consults with our other physicians when they need an experienced second opinion.

Outpatient Rehabilitation
Our team of highly-experienced professionals offers expertise in physical therapy, occupational therapy, hand therapy and speech therapy.
