Getting a Copy of Your Medical Record
In order to obtain copies of your medical record, you must submit a HIPAA compliant release form to the Medical Records Department. This may be done in different ways:

1. Request a form by phone. Call the Medical Records Department at 303-415-7760 and request that a form be mailed to you.

2. Request a form in person. To obtain a form in person, please visit our Medical Records Department, located on the BCH Foothills Campus in the Tebo Family Medical Pavilion at 4715 Arapahoe, Suite G30, Boulder, CO 80303. Office hours are Monday-Friday, 8 a.m. to 5 p.m.

Please note that there is a charge for personal copies of medical records. This charge is determined by the Colorado Department of Public Health.    

Making a Change to Your Medical Record
For your privacy, any updates or corrections to your medical record must be requested in person with a photo ID. Changes cannot be made over the phone. The Medical Records Department is located on the Foothills Medical Campus in the Tebo Family Medical Building at 4715 Arapahoe, Suite G30, Boulder, CO 80303.
