In order to obtain copies of your medical records, you must submit a HIPAA compliant release form to the Medical Record Department. This may be done in different ways: 1. Phone the Medical Record Department at 303-440-2079 and request that a form be mailed to you. You may also come to the Medical Record Department and obtain a form. 2. CLICK HERE to print out a PDF version of the form. Then FAX or mail to: Medical Record Department Boulder Community Hospital PO Box 9019 Boulder, Colorado 80301-9019 FAX: 303-440-2359 Please note that there is a charge for personal copies of medical records. This charge is set by the Colorado Department of Public Health. The charge for your personal copies is $14.00 for the first 10 pages, 50 cents per page for pages 11-40, and 33 cents per page for pages 41+.
