BCH's Occupational Health Services (OHS) is the most comprehensive provider of occupational medicine in Boulder County. The OHS clinic is conveniently located at the Community Medical Center (CMC) in Lafayette:

Designating OHS is easy!  Simply download the forms below, indicate the care choices that are right for your business, and either fax or e-mail them back to us.  Instructions are on each page.   Then, contact your Work Comp Insurance Provider and let them know that you have selected us as your Work Comp Care Provider.

Where do you go if you are injured on the job?

Always notify a supervisor immediately, or as soon as possible, so they can authorize treatment. Once treatment has been authorized (or if your supervisor is not immediately available), call 303-415-4340 for an appointment.

For minor injuries during the hours of 8:30 am -4:00 pm, Monday - Friday, you may contact our office at 303-415-4340 and let us know if you are on your way.  Although we will work to accommodate walk-in patients, we ask that, whenever possible, injured employees call for an appointment at 303-415-4340 so that we can ensure the most appropriate and convenient care. 

For life threatening injuries, call 911 and ask to be taken to one of the Boulder Community Health facilities.

    * Drug testing must be completed within 24 hours after an injury and there must be a Drug Testing Contract on file at the lab signed by the Employer.
